I have a 78 year old active patient with lumbar spondylosis/radiculopathy and a proximal HS tendinopathy/partial tear as noted on MRI. She has had chronic pain for years with some atrophy at the gastroc. Notable HNP in her 40s treated with traction. Most of her pain is at the ischial tuberosity. I would like to incorporate some dry needling and had planned to focus on the L345/piriformis/inf gluteal/sup cluneals/HS/sural points but am unsure about inserting a needle at the ischial tuberosity due to the partial tear. Thoughts?
Hi Gwen! Sorry for the delay here. Based on what you have written, I don’t have any issues inserting needles at the partial tear of the hammy. I would treat as you have outlined! Hope that is helpful!
